If you need a transcript, complete this Release of Records form and send it with $5 transcript fee to:

Our office is open for transcripts Mondays through Thursdays, 9:00am to Noon.
Please note that an “Official” transcript must be sent directly from our office to the recipient (usually a higher education institution), so be sure to include all recipient name and address information.
Alternatively, an Official transcript may be given to the student in a sealed, tamper-evident envelope.
Only an “unofficial” transcript may be given or mailed to the student.
